Amusement Park Rides

APPLYING FOR A CERTIFICATE OF OPERATION

Submit to the Secretary of State the following:

(Note: All documents and payment must come as a complete package - sending separate documents will delay your approval)

1. A list of all rides that will be used in this state, clearly stating the proper ride model, serial number and passenger capacity of the ride, the recommended maximum speed of the ride, and recommended direction of travel of the ride.

2. A Certificate of insurance in the amount of not less than $1,000,000.00 which insures both the owner and the operator against liability for injury to persons and property arising out of the use or operation of the amusement ride.

3. Payment of the fee in the amount of $100.00. payable to the Vermont Secretary of State.
